Nursing Case Management

Nursing case management is a field in nursing that entails four steps; 1) assessing the client, 2) planning the care for the client, 3) implementing the plan of care, and 4) evaluating the effectiveness of the plan. Nurse case managers work in a collaborative manner with other healthcare professionals, the caregivers, the family members, and (of course) the client.
